Welcome to the CINC Family! We will need to gather some information before your Project Call 1 with your Implementation Manager. Project Call 1 will determine when your site launches.

Who is filling out this form? First and Last Name of CINC Site Owner License # Phone Number to Display on Site (Do Not list your cell if you do not want it displayed to the public) *

Office Address to Display (This MUST be your Brokerage's Office Address for compliance) *

City, State, Zip Team Name if Applicable Brokerage Name MLS Names/Board Affiliations (no abbreviations). Please note, Flex MLS and Paragon are data providers, not MLS boards.

FlexMLS User ID (only applicable if your MLS uses Flex as a data vendor).

Broker of Record (First & Last Name) Broker of Record Email (Required for MLS Compliance. Please DO NOT list your email address if you are not the BOR) *
